Sepsis, a life-threatening condition, causes tens of millions of fatalities annually, making early diagnosis a significant hurdle. The diagnostic potential of microRNAs (miRNAs) for sepsis, specifically miR-155-5p, miR-21, miR-223-3p, miR-146a, and miR-125a, has been the focus of many studies in recent years. Accordingly, we conducted this meta-analysis to determine if microRNAs can be employed as biomarkers in the identification of sepsis cases.
From PubMed, Cochrane Central Register of Controlled Trials, EMBASE, and China National Knowledge Infrastructure, we culled relevant data up until May 12, 2022. A fixed/random-effects model meta-analysis was accomplished using software packages Meta-disc 14 and STATA 151.
A total of fifty pertinent studies were scrutinized during the analysis. In a combined assessment of miRNA detection performance, the sensitivity was 0.76 (95% confidence interval [CI] 0.75–0.77), the specificity was 0.77 (95% CI 0.75–0.78), and the area under the summary receiver operating characteristic curve (SROC) was 0.86. A subgroup analysis revealed that miR-155-5p demonstrated the greatest area under the receiver operating characteristic (ROC) curve (AUC) among all pooled miRNA sensitivities, measuring 0.71 (95% confidence interval [CI], 0.67 to 0.75); pooled specificity, 0.82 (95% CI, 0.76 to 0.86); and ROC curve performance, 0.85. Across the four microRNAs—MiR-21, miR-223-3p, miR-146a, and miR-125a—SROC values were 0.67, 0.78, 0.69, and 0.74, respectively. The meta-regression analysis showed that the specimen type was responsible for the observed heterogeneity. The SROC value for serum surpassed that of plasma, measuring 0.87 versus 0.83, respectively.
Based on a meta-analysis of multiple studies, it was found that miRNAs, in particular miR-155-5p, may serve as potentially helpful indicators for sepsis detection. Diagnostic procedures often include a clinical serum specimen as a critical element.
